Stewards used synthetic vinyl gloves and were exposed to skin irritation hazards.    b) EDR Employee Dining Area: Line cooks were not provided appropriate gloves such as wire mesh gloves to prevent finger amputation hazards while handling knives and meat slicers. Line cooks used synthetic vinyl gloves and were exposed to finger amputation.    c) Maintenance Building: Green welding goggles and a welding face shield were not maintained in a sanitary condition.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    Facility Wide: Energy control procedures (lockout/tag out), periodic audits, and training were not provided. All required elements of an energy control procedures  were not understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were not acquired by employees performing servicing, testing, inspecting, repairing, and maintaining work on a machine or equipment throughout the facility, where the unexpected release of stored energy could occur and cause injury, equipment shall be isolated from the energy source.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  Facility Wide: Employer failed to report an in-patient hospitalization of a burn related injury occurring within 24 hours of a work related incident. On February 11, 2017, around 6:50 pm, an employee carrying corrosive material spill on his pants while performing his duties at Table Mountain Casino. The incident was reported to OSHA on February 15, 2107, at 2:50 pm.
