The path to ground from circuits, equipment, and enclosures was not permanent and continuous.      a/ Lita Brothers Construction Edison, NJ site - Ground level: 3 prong extension cord, attached to a drill,  was missing grounding pin and in use by employees on the roof.      Observed on or about 04/20/2012.

Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrailsystem, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.     a/ Lita Brothers Construction Edison, NJ site - Warehouse exterior roof: Employees performing roofing operations 24 feet in height without approved fall protection devices.
