29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a)  1611 Springfield Avenue, Maplewood, NJ.  Employees were exposed to falls of up to 12 1/2 to 18 1/2 feet to the sidewalk canopy or to the ground below as the platform on the working level of the scaffold was not fully planked.   Violation observed on or about 08/06/2015.

Employees were exposed to injury through a scaffold collapse as concrete blocks, which are not an adequate foundation, were used to support the scaffold on its West side. 29 CFR 1926.451(g)(1)(vii):   Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a)  1611 Springfield Avenue, Maplewood, NJ.  Employees were exposed to falls of up to 12 1/2 to 18 1/2 feet to the sidewalk canopy or to the ground below as guardrails were missing and personal fall arrest systems were not in use.    Violation observed on or about 08/06/2015.   Lita Brothers Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1011704, citation number 1, item number 3, and was affirmed as a final order on June 4, 2015, with respect to a workplace located at 1611 Springfield Avenue, Maplewood, New Jersey.

Subcontractor employees were exposed to falls of 10 to 11 feet to the floor below as the top edge height of the guardrail was only 37 inches which allows for an "over-the-railing" fall. LITA BROTHERS CONSTRUCTION WAS PREVIOIUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 981796, CITATION NUMBER 1, ITEM NUMBER 4, AND WAS AFFIRMED AS A FINAL ORDER ON 06/04/2015, WITH RESPECT TO A WORKPLACE LOCATED AT SPRINGFIELD AVENUE AND TUSCAN ROAD, MAPLEWOOD, NJ.
