29 CFR 1910.212(b):     Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving.    Machine shop:  On or about May 1, 2012 and times prior thereto, a drill press was not secured from tipping.  This exposed to struck-by hazards from the drill press tipping while drilling.

29 CFR 1915.56(b)(2):     Only cable free from repair or splices for a minimum distance of ten (10) feet from the cable end to which the electrode holder is connected shall be used, except that cables with standard insulated connectors or with splices whose insulating quality is equal to that of the cable are permitted.    Red Barn:  On or about May 1,  2012 and times prior thereto, the stinger connected to Miller welding machine had a cut to the condutor at 68 inches.  This exposed employees to electric shock hazards if they came in contact with the conductor during arc welding operations.

29 CFR 1915.131(f):     The moving parts of drive mechanisms, such as gearing and belting on large portable tools, shall be adequately guarded.    a)  Machine shop:   On or about May 1, 2012 and times prior thereto,  the Summit lathe did not have a chuck guard installed.   This exposed employees to caught in hazards if their clothes or body was caught in the chuck.    b)  Machine shop:  On or about May 1, 2012 and times prior thereto,  Bradford mill press  S/N 800095 had unguarded belts and pulleys.  This exposed employees to caught-in/amputation hazards to the hands.     c)  Machine shop:  On or about May 1, 2012 and times prior thereto, a Hallidie drill press had unguarded belts and gears and shafts exposed.  This exposed employees to caught-in/amputation hazards to the hands and clothes during drill press operations.

29 CFR 1910.213(d)(1):     Each circular crosscut table saw shall be guarded by a hood which shall meet all the requirements of paragraph (c)(1) of this section for hoods for circular ripsaws.    Wood shop:  On or about May 1, 2012 and times prior thereto,  the power matic table saw S/N 8466614 did not have a hood guard installed.  This exposed employees' to cut and amputation hazards.

29 CFR 1915.134(b):     Floor and bench mounted grinders shall be provided with work rests which are rigidly supported and readily adjustable. Such work rests shall be kept a distance not to exceed 1/8 inch from the surface of the wheel.    Machine/Fabrication shop:  On or about May 1, 2012 and times prior thereto, the bench grinder tool rest measured at 7/8 inches.  This exposed employees to struck-by hazards if a tool or work piece got caught between the wheel and work rest causing the wheel to break.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    Machine shop:  On or about May 1, 2012 and times prior thereto, the Milwaukee heavy duty drill S/N 0073985237 had defective strain relief.  This exposed employees to shock hazards from a damaged cord.    Machine shop:  On or about May 1, 2012 and times prior thereto, the Dewalt grinder had  damaged strain relief.  This exposed employees to shock hazards from a damaged cord.

29 CFR 1915.507(b)(1):     The employer must select, install, inspect, maintain, and test all portable fire extinguishers according to NFPA 10-2002 Standard for Portable Fire Extinguishers (incorporated by reference, see  1915.5).    a) On or about May 1, 2012 and times prior thereto, none of the fire extinguishers in the facility had monthly inspections completed.  This exposed employees to burns and smoke inhalation from the possibilty of an fire extinguisher not being servicable.    b) Machine shop:  On or about May 1, 2012 and times prior thereto, a fire extinguisher was blocked.  This exposed employees to smoke inhalation hazards from delays in accessing the fire extinguisher.
