29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  Machine and aluminum shops, on or about 15 July 2015 and times prior thereto; two instances were noted where energized electrical outlets were damaged:       a. Machine shop, northeast corner; an electrical outlet had damage on the top receptacle.        b. Aluminum shop, south wall; an electrical outlet had damage on the lower receptacle.   This condition exposed employees to an electrical shock and burn hazards.

29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity.  Yard shop; on or about 15 July 2015 and times prior thereto; a Magic Chef refrigerator make and model unknown was plugged into an energized electrical outlet that was wired with reverse polarity. This condition exposed employees to electrical shock and burn hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  Various facilities; on or about 15 July 2015 and times prior thereto; four instances were noted where electrical openings were not effectively closed:       a. Machine shop adjacent to milling machine; knockout plugs were missing on the bottom left and upper right hand sides on an energized outlet.       b. Machine shop adjacent to bay roll-up door; a knockout plug was missing from the upper right hand side of an energized outlet.       c. Counter area, adjacent to main entrance; a knockout plug was missing on the center plug on the right hand side of an energized outlet.         d. Building 916B, wood shop area; and energized electrical panel was missing a blank in breaker position number 1  This condition exposed employees to electrical shock and electrical burn hazards.

29 CFR 1915.134(b): Work rests were not kept a distance not to exceed 1/8 inch from the surface of the wheel.  Yard shop; on or about 15 July 2015 and times prior thereto; the work rest on a Baldor bench grinder, serial number 1184, was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured 1/4 of an inch. This condition exposed employees to a caught in hazard.     Piston and Rudder, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1915.134(b), which was contained in OSHA inspection number 419043, citation number 1, item number 4 and was affirmed as a final order on 11 July 2012, with respect to a workplace located at the Machine/fabrication shop.

29 CFR 1915.72(a)(1): When ladders with broken or missing rungs or steps, broken or split side rails, or other faulty or defective construction were discovered, they were not immediately withdrawn from service.  Yard area; on or about 15 July 2015 and times prior thereto; the employer failed to remove damaged six foot wooden ladder from service. Ladder had a damaged rear rail. This condition exposed employees to a fall hazard.
