29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): Wood Shop: On or about July 15, 2015 and at times prior thereto, a five (5) foot wide unguarded edge was present adjacent to the upper storage access. This condition exposed employees to a fall hazard of seven (7) to 13 feet.

29 CFR 1915.116(b): Loads were not safely rigged before being moved: (a) Yard Area, P&H Omega Crane: On or about July 15, 2015 and at times prior thereto, a landing craft (AK 5200 M) was rigged and elevated by the four (4) mooring bits on the port and starboard sides of the vessel. This condition exposed employees to struck-by hazards. (b) Yard Area, P&H Omega Crane: On or about July 15, 2015 and at times prior thereto, a landing craft (AK 5200 M) was rigged and elevated the main hoist was not centered over the load. This condition exposed employees to struck-by hazards. (c) Yard Area, P&H Omega Crane: On or about July 15, 2015 and at times prior thereto, a landing craft (AK 5200 M) was rigged and elevated without the extended outriggers padded or cribbed to prevent soil displacement. This condition exposed employees to struck-by hazards.
