29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.      North wall, east end of interior building, north central area of worksite: On 05/10/2012, employees were exposed to fall hazards while standing upon an unguarded scaffold platform at heights greater than 10 feet above the ground, and not wearing a personal fall arrest system. Employees were standing upon the unguarded scaffold while engaged in site welding activities.

29 CFR 1926.760(a)(1):     Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    Center interior area of building, north central area of worksite, fall protection: On 05/10/2012, employees engaged in steel erection activities were exposed to fall hazards whlle sitting upon an installed roof support beam and not wearing a personal fall arrest system. The installed roof support beam used by employee to sit upon was located 16 feet, 3 inches above the ground.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Center interior area of building, north central area of worksite, portable ladder: On 05/10/2012, employees engaged in steel erection activities were exposed to fall hazards using a portable ladder whose side rails were not extended at least 3 feet above the upper landing surface being accessed. The side rails of the ladder used by employees to access the upper landing surfaces extended 18 inches above the landing surfaces whose height above the ground was 16 feet, 3 inches.
