29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Northern Tool + Equipment: On or about January 25, 2019, employees without fall protection were exposed to a fall hazard greater than 15 feet, while installing metal decking without being trained on fall hazard recognition.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: Northern Tool + Equipment: On or about January 25, 2019, and at times prior thereto, employees were exposed to a fall hazard greater than 15 feet while installing metal decking on the roof without using a personal fall arrest system or other fall protection alternative.
