29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. At a commercial steel erection site, employees engaged in welding activities were accessing the elevated level of the structure from a portable ladder that did not extend at least 3 feet above the landing exposing the employees to fall hazards of approximately 19 feet to the ground below.

General-duty citation text
29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. At commercial steel erection site, employees working at elevated heights without fall protection were exposed to fall hazards of approximately 19 feet. JP Steel Erectors was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 428531, citation number 1, item number 2 and was affirmed as a final order on October 30, 2012, with a final abatement date of October 5, 2012 with respect to a workplace located at 2314 East Expressway 83, Mission, TX 78752.
