29 CFR 1918.24(i)(1): The employer did not maintain portable ladders in safe condition. The Tuna Fishing boat Koorale unloading tuna fish at the Starkist Tuna processing plant and employees were using defective and broken aluminum ladders during the unloading of the vessel on 05/7/2012. Some of the broken ladders had been repaired with a piece of aluminum stock and then duct-taped while others exhibited sharp torn edges where the ladders had broken into. Employees could have cut themselves while going up and down the ladders or fallen.

29 CFR 1918.24(i)(2): Ladders were not inspected for defects before each use. The Koorale was unloading tuna fish at the Starkist terminal and employees were accessing the fish wells using aluminum step ladders that had not been inspected prior to use. The ship had no ladder inspection procedures.
