29 CFR 1926.503(c)(3):     The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:    Repeat: The (Covington Roofing and Sheet Metal) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(c)(3), which was contained in OSHA inspection number 315041293, citation 1, item number 2 and was affirmed as a final order on (01/20/11), with respect to a workplace located at 109 Wrights St, Hot Springs, Arkansas.    instance a:  On or about May 25, 2012, east edge of roof, the employer did not ensure affected employee's including, but not limited to foreman, safety monitor, roofers  retained the required understanding, knowledge, and skill of using a warning line system. The warning line was not completely erected around all sides of the roof area where foreman and employees were performing construction demolition and installation of new roof system. This created a condition which exposed employees to the hazards of falls from an elevation of approximately 9-1/2-feet from roofline to next lower level.    instance b:  On or about May 25, 2012, on the roof, the employer did not ensure that the safety monitor retained the required understanding, knowledge, and skills of performing his responsibilities. The safety monitor was distracted from his/her monitoring activities by performing such tasks as shoveling roof material from roof and into a wheelbarrow. This created a condition of which exposed employees to the hazard of falls from an elevation of approximately 9-1/2-feet from roofline to next lower level.
