29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     Monarch Metal Manufacturing Inc, at 2597 West 64th Avenue, Denver, CO: On and preceding 5/31/12, Monarch Metal Manufacturing Inc did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Employees MIG and TIG welded carbon steel, stainless steel, and aluminum.  Employees used portable hand grinders to remove metal burrs.  Employees walked through the welding and forming shops during fabrication of metal products.  The employer did not ensure that employees wore appropriate eye protection while working in the shops.  This condition exposed employees to a flying object hazard.

29 CFR 1910.136(a): The employer did not ensure that each affected employee use protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:    Monarch Metal Manufacturing Inc, at 2597 West 64th Avenue, Denver, CO: On and preceding 5/31/12, Monarch Metal Manufacturing Inc did not ensure that each affected employee use protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards.  Employees MIG and TIG welded carbon steel, stainless steel, and aluminum.  Welded products weighed approximately ten to twenty pounds.  The employer did not ensure that employees used protective footwear.  This condition exposed employees to a falling object hazard.

An employee "fused" steel flanged nuts to carbon steel.  The flanged nuts contained lead.  This process generated lead fume.  On 5/31/12 one employee was exposed to lead at a concentration of 0.088 mg/m3 as an eight hour Time Weighted Average (TWA).  This is 1.76 times the PEL.  Air sampling was conducted for 461 minutes.

Employees MIG and TIG welded carbon steel, stainless steel, and aluminum.  Welding carbon steel or aluminum generated aluminum fume and welding fumes.  The employer did not evaluate employee's exposure to the abovementioned air contaminants.  This condition potentially exposed employees to a respiratory hazard.
