29 CFR 1910.1200(e)(1):  The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:          (a)    Monarch Metal Manufacturing, Inc. at 550 W 53rd Place, Denver, CO 80216: The employer did not develop, implement, and maintain at the workplace specific written hazard communication program which describes how the specified in paragraphs (f), (g), and (h) of this section for labels of warning, safety data sheets, and employee information and training will be met. The employer did not update their program to reflect changes made to this standard in 2012.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.134(d)(1)(iii):  The employer did not evaluate the respiratory hazard(s) in the workplace including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminants chemical state and physical form:     (a)    Monarch Metal Manufacturing, Inc. at 550 W 53rd Place, Denver, CO 80216: On or before March 31, 2016 the employer did not assess employees exposure to welding fumes, including but not limited to hexavalent chromium.
