Condition exposed employee to the hazard of a serious head injury.

(a)    Monarch Metal Manufacturing, Inc. at 550 W 53rd Place, Denver, CO 80216:  On or before October 21, 2015 the exposing employer did not inspect the energy control procedure at least annually. Documents provided by Monarch Metal Manufacturing, Inc. document the inspection of the energy control program in 2010.  The equipment includes machines with multiple energy sources such as press brakes.  Condition exposed employees performing energy control procedures to potential unexpected machinery start ups or other hazards due to changes in the various pieces of machinery being serviced since the last review of the energy control program.
