The damage included rope distortion and broken wires. This exposed the employees to being struck by falling loads or swinging tongs upon failure of the wire rope.

(b) Xtreme Drilling and Coil Services, Pergamos #8-41-7-60, Rig #19, Briggsdale, CO 80611: On or about May 22, 2012, the employer failed to provide guardrails on the rig floor. There were gap openings of 11-13 inches wide on the left and right side of the steps leading down from the rig floor to the draw works level.  This condition exposed employees to fall hazards of over 4 feet.
