29 CFR 1910.23(a)(8): Floor hole(s), into which persons could accidentally walk, were not guarded by standard railings with standard toeboards on all exposed sides or, by floor hole covers of standard strength and construction: (a)Xtreme Drilling and Coil Services, Rig 24, Platteville, CO: On or about April 13th, 2015, the employer did not ensure an appropriate size cover to prevent employees from accidentally stepping into the cellar pit. The cover around the well bore on API #05-123-40990 measured an 8 inch space between the cover and well cap. (b)Xtreme Drilling and Coil Services, Rig 24, Platteville, CO: On or about April 13th, 2015, the employer did not ensure an appropriate size cover to prevent employees from accidentally stepping into the cellar pit. The cover around the well bore on API #05-123-40989 measured a 5 inch space between the cover and well cap. (c)Xtreme Drilling and Coil Services, Rig 24, Platteville, CO: On or about April 13th, 2015, the employer did not ensure an appropriate size cover to prevent employees from accidentally stepping into the cellar pit. The cover around the well bore on API #05-123-40991 measured a 5 inch space between the cover and well cap. 29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): (a)Xtreme Drilling and Coil Services, Rig 24, Platteville, Colorado: On or about April 6th, 2015, the employer did not ensure employees were protected from falls from an elevated platform over the Scavenger/Shale Shaker unit. As a result, an employee was seriously injured after falling approximately 7'-6" to the ground below. The injured employee had been using a hammer to loosen a 10 inch flow line hammer union when he fell from the open sided platform.
