29 CFR 1910.178(n)(7)(i): When ascending or descending grades in excess of 10 percent, loaded trucks shall be driven with the load upgrade. a) Xtreme Drilling and Coil Services, County Road 4, Rig #6, Hudson, CO 80642: On or before June 26, 2015 an employee operated a John Deere loader, model 544K, serial number SN 1DW544KZCCE647800 to a move a casing joint from the casing rack to the slide transition. The loader was operated on a surface that was not level causing the pip to roll off of the front of the loaders tines, striking an employee, resulting in a compound fracture. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
