OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to amputation hazards associated with rotating machinery:    (a)  Xtreme Drilling and Coil Services, Rig 6 at Hudson, Colorado: On or about June 2, 2015, the employer did not protect employees from hazards associated with rotating parts on the two Pull-Master hydraulic winches.  While standing in the derrick and unspooling a 9/16 inch wire rope from winch #2, an employee was exposed to the unguarded rotating winch.  This condition exposed employees to caught-in and amputation hazards from contact with rotating parts.       Abatement Note:  Among other methods, one feasible and acceptable method to correct the hazard is to not allow employees on the derrick when servicing the winch unless the winch is isolated in accordance with the Lock-out/tag-out policy XDCS-QHSE 13046.
