29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) At the front of the house, 2 x 4 lumber with protruding nails lay within two feet of a pathway to other work areas on the ground level. The lumber was used as formwork and dismantled several days prior to this inspection. Employees were exposed to the hazards of nails impaling their feet.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: (a) On June 6, 2012, or at times prior thereto, the controlling and exposing employer Island Corner Stone, failed to ensure the use of appropriate gloves while mixing and applying Portland cement. The Portland cement was mixed with water in a five gallon bucket and applied onto CMU blocks. The exposed employee was observed handling/applying Portland cement with his bare hands.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. (a) At the project on or about June 6, 2012, and at times prior thereto, the "Hubbel Portable Circuit Guard" ground-fault circuit-interrupter was inoperative. There was no assured equipment grounding conductor program when extension cords were used to power a 110 VAC DeWalt chopsaw, Model D28715. The chopsaw was used to cut 2 1/2" metal studs.
