29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not  protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502:    a)  On February 25, 2015, on the roof on the front of the house, fall protection was not provided for employees filling imperfections in the concrete with concrete and smoothing it out with a trowel, exposing employees to the hazard of falling from a height of 11 feet, 1 inch to the ground below.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a)  On February 25, 2015, in front of the house, several pieces of formwork on the ground with nails that protruded about 1.5 inches from the lumber.  The formwork with nails was located in the passageways from the parking area to the house.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not  protected from damage:     a)  On February 25, 2015, in the driveway and parking area of the project, a 110 VAC extension cord, used to power a 110 VAC Makita right angle grinder, model 9557PB, was not protected from damage as it lay on the compacted gravel and was run over by vehicles.
