29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On January 10, 2017, inside the proposed living room, a 50 foot 12/3 AWG extension cord with damaged strain relief to the outer insulation was used to supply power to a Boss PE16C Electro Mechanical Mixer.

On January 10, 2017, at the garage area, an employee working with wet Hanson Portland cement was not provided with nitrile or otherwise chemical-resistant gloves when working with wet Portland cement.
