29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not be protected by protective helmets: (a) Near the school's Main Office on or about June 19, 2012, and at times prior thereto, hard hats were not worn by employees at ground level that handed up corrugated metal panels to employees on the walkways' roofs, located 8 feet, 3 inches above the ground. Employees installed new corrugated metal on the walkways' roofs.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: (a) Near the school's Main Office on or about June 19, 2012, and at times prior thereto, no ladder, stairs, ramp, runway, sloped embankment or personnel hoist was provided to access the walkways' roofs, located 8 feet, 3 inches above the ground. Employees climbed the end frames of tubular welded frame scaffolds to 5 foot, 6 inch high platforms and then climbed to the walkways' roofs. Employees installed new corrugated metal on the walkways' roofs.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used: (a) Near the school's Main Office on or about June 19, 2012, and at times prior thereto, safe access was not provided to the scaffolds' platforms as employees climbed the end frames of tubular welded frame scaffolds to 5 foot, 6 inch high platforms and then climbed to the walkways' roofs. Employees installed new corrugated metal on the walkways' roofs.
