29 CFR 1926.1424(a)(2)(ii): The employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas surrounding the cranes superstructure: Capital City Group, Inc. on or about June 15, 2012 at the site located at I-75 & Fourth Street, Lima, Ohio: The employer failed to ensure the counterweight swing radius of the Leiberh 300 ton Crane was adequately barricaded, marking the boundaries of the hazard area. An employee was exposed to stuck-by and crushed-by hazards.
