29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    a.  At the site, employees were installing brick on a chimney from a tubular welded frame scaffold that was not properly planked on the platform were the employees were working, thereby exposing the employees to a fall hazard of 36 feet.

29 CFR 1926.451(g)(3)(ii):     When horizontal lifelines are used, they shall be secured to two or more structural members of the scaffold, or they may be looped around both suspension and independent suspension lines (on scaffolds so equipped) above the hoist and brake attached to the end of the scaffold. Horizontal lifelines shall not be attached only to the suspension ropes.    a.  At the site, employees were installing brick from a tubular welded frame scaffold.  The employees were using a 5/8" rope as a horizontal lifeline that was secured to a single vertical member of the scaffolds frame.  The single vertical section of the scaffold does not have the capacity to be used as an anchor for fall protection, thereby exposing the employees to a fall of approximately 36 feet.

29 CFR 1926.451(g)(4)(i):     Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a.  At the site, employees were installing masonry brick on a chimney from a tubular weld frame scaffold that had scaffold planks that extended perpendicular from the scaffold onto a steep roof.  The make-shift work platform did not have a guard rail system installed on the inner portion of the work platform, thereby exposing the employees to a fall hazard of approximately 36 feet.

29 CFR 1926.452(c)(5)(iii):     Brackets used to support cantilevered loads shall:    Be used only to support personnel, unless the scaffold has been designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of the scaffold.    a.  At the site, employees were installing brick from the side brackets of a tubular welded frame scaffold that had scaffold planks that extended perpendicular from the scaffold bracket's planks, which increased the tipping forces of the scaffold, thereby exposing the employees to a fall hazard of approximately 36 feet.

29 CFR 1926.502(d)(6)(i):     Unless the snaphook is a locking type and designed for the following connections, snaphooks shall not be engaged:    directly to webbing, rope or wire rope;    a.  At the site, employees were installing masonry brick from a tubular weld frame scaffold. There was an employee working from this scaffold who tied an end-of-the-line bowline on a 5/8" synthetic rope to secure a snap hook from a lanyard to be used as a part of a fall protection system.  Fall protection used in this manner exposed the employee to a fall hazard of approximately 36 feet.
