Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential struck-by hazard: (a) On or about 04/16/2012, and at times prior to that date, for employees working in the tank department and exposed to the potential hazard of being struck by a pressure vessel suspended by the overhead crane. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1. Retrofit the overhead cranes with radio controls to eliminate the wired pendents and remove the operator from close proximity to the suspended load. 2. Use tethers attached to suspended loads to minimize to the best extent possible the potential swinging motion of suspended loads.
