29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: At the bottom of the stairway to the rig floor at the shale shakers on the top of the mud tanks, employees working in and around the top of the mud tanks and shale shakers were exposed to trip and fall hazards from two unguarded floor holes which measured approximately 4 inches in width.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: At this drilling rig location, employees live and work with electrical equipment and the the electrical disconnects were not identified as to their purpose or function such as but to limited to the following areas: 1. On the Day Crew Trailer in the electrical panel on the outside wall, there were three unidentified circuit breakers; 2. On the Night Crew Trailer in the electrical panel on the outside wall, there were twenty unidentified circuit breakers. 3. On the Rig Managers Trailer in the electrical panel on the outside wall, there were nineteen unidentified circuit breakers.
