29 CFR 1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:      a.) Heath Oil, Inc. 5809 State Route 8, Harrisville, PA 16038, on or about July 18, 2012, and times prior thereto:  Company employees were working with and around hazardous substances including but not limited to:  CRC Brakleen Brake Parts Cleaner and CRC Power Lube Multi-Purpose Lubricant.  Employer did not ensure that new employees were trained in Hazard Communication Program training prior to beginning work on site.

Heath Oil, Inc. 5809 State Route 8, Harrisville, PA 16038,-Southwest corner of the Maintenance garage- on or about July 18, 2012, and times prior thereto:  Clear, quart-sized, plastic spray bottles, were not labeled to identify the hazard warnings, or the identity of the chemical(s) contained within the containers.

29 CFR 1910.106(e)(9)(iii):  Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily:    a.) Heath Oil, Inc. 5809 State Route 8, Harrisville, PA 16038,-Northeast corner of Maintenance garage- on or about July 18, 2012, and times prior thereto:  Company employees used cloth rags for the purpose of cleaning up flammable and/or combustible liquids including, but not limited to, used vehicle oils and lubricants.  A metal receptacle for disposal of the used oil and lubrication soaked rags was not provided.
