Heath Oil, Inc. dba All American Gasoline & Car Wash, 18136 Conneaut Lake Road, Meadville, PA 16335, on or about December 20, 2019 and times prior thereto: A circuit breaker panel box that powered equipment including, but not limited to, heaters, lights and the time clock, had unused openings that were not effectively closed. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The employer did not maintain a hazard communication program on-site.
