Heath Oil, Inc., 5609 Pennsylvania State Route 8, Harrisville, PA  16038, mechanic's garage area, on or about February 7, 2020, and times prior thereto:  Employees operate a Caterpillar powered industrial truck (M/N:  DP30, S/N:  7BM00625) for purposes including, but not limited to, moving equipment.  The windshield of the powered industrial truck was broken in the operators field of view.    B.)  Heath Oil, Inc., 5609 Pennsylvania State Route 8, Harrisville, PA  16038, mechanic's garage area, on or about February 7, 2020, and times prior thereto:  Employees operate a Caterpillar powered industrial truck (M/N:  DP30, S/N:  7BM00625) for purposes including, but not limited to, moving equipment.  The right window of the powered industrial truck was broken and posed a laceration hazard.     C.)  Heath Oil, Inc., 5609 Pennsylvania State Route 8, Harrisville, PA  16038, mechanic's garage area, on or about February 7, 2020, and times prior thereto:  Employees operate a Caterpillar powered industrial truck (M/N:  DP30, S/N:  7BM00625) for purposes including, but not limited to, moving equipment. The use of the GE relocatable power tap (power strip) did not meet listing and labeling requirements in that the relocatable power tap is intended to be directly connected to a permanently installed branch-circuit receptacle outlet, and are not intended to be series connected (daisy chained) to other relocatable power taps.
