At the job site, 800 Hayes Dr. Manhattan, Kansas, employees were exposed to ingoing nip points from the unguarded lower end belt drive for the glass sander.  Employees were exposed to caught in hazards.

29 CFR 1910.219(f)(3):  Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    At the job site, 800 Hayes Drive, Manhattan, Kansas, Employees were were exposed to ingoing nip points from the unguarded chain and sprocket drivers on both sides of the Glass Washer.  Employees were exposed to caught in hazards.

29 CFR 1910.219(d)(1):  Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    At the job site, 800 Hayes, Manhattan, Kansas, employees working in the tempering department were exposed to a drive pulley that was not guarded at the top.  Employees were exposed to in-going nip points and amputation hazards from unguarded pulley.       Manko Window Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.219(d)(1), which was contained in OSHA inspection number 312799018, citation number 1, item number 4A and was affirmed as a final order on 5 May, 2009, with respect to a workplace located at 800 Hayes Dr. Manhattan, Kansas 66502.

Employees were exposed to amputation hazards from ingoing nip points.
