29 CFR 1910.22(b)(1):     Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:    North side of this metal plating facility, across the shop floor from the restrooms, the elevated walkway metal flooring near the eyewash/shower station was not maintained in good repair.  The walkway contained holes and openings which exposed employees to slips, trip, and falls.

29 CFR 1910.303(f)(2):     Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    South side of this metal plating facility, the electrical panels inside the south side bay door did not have the circuit breaker switches labeled or identified as to which circuit they controlled in the following instances:     a.  Panel #1, 110 volts,     b.  Panel #2, 240 volts, and    c.  Panel #3, 480 volts.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    An electrical box which housed a previously removed timer was left open continuously near the elevated walkway on the north side of this facility.  The box was mounted on an I-beam column and was less than 8 feet above the floor.  Employees were exposed to live conductors.
