29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    On or about July 26, 2012:    Employees were exposed to fall hazards while engaged in residential construction activities of the following instances:    a) Employee was exposed to fall hazards of approximately 11. 4 while prepping an exterior wall on the second floor, while standing at the edge of the second floor area without use of fall protection means.    b) Employees were exposed to fall hazards of approximately 10.3 feet while working on the second floor around an opening in the floor where steps were to be installed.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about July 26, 2012:    Employer failed to implement and maintain a training program for employee exposed to fall hazards in the following instances:    a) Employee was exposed to fall hazards of approximately 11. 4 while prepping an exterior wall on the second floor, while standing at the edge of the second floor area without use of fall protection means.    b) Employee were exposed to fall hazards of approximately 10.3 feet while working on the second floor around an opening in the floor where steps were to be installed.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about July 26, 2012:    Employee were exposed to fall hazards of approximately 10.3 feet while trying to access second floor area, by using a multi-purpose ladder in where the rails of the ladder did not extend above the upper landing surface at least three feet.
