29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about August 15, 2022, at the above addressed jobsite, employees performing roofing operations on a house, approximately twenty-four (24) feet two (2) inches,  above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about August 15, 2022, at the above addressed jobsite, employees performing roofing operations on a house, approximately twenty-four (24) feet three (3) inches,  above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  c.  On or about August 15, 2022, at the above addressed jobsite, an employee performing roofing operations on a house, approximately twenty-three (23) feet nine (9) inches,  above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR  1926.502(d)(17):The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a.  On or about August 15, 2022, at the above addressed jobsite, employees conducting residential roofing operations was not wearing personal fall arrest system (PFAS)  with the attachment point at the employee's back, at a fall height for twenty three (23) feet nine (9) inches above the ground, thereby exposing the employee to the hazards associated with falls.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a.  On or about August 15, 2022, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
