29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel. (a) Outside south-west side of Warehouse, left of loading ramp/door area: On or about 3 August 2012 and times prior thereto, the 120 volt, Ground Fault Circuit Interrupter was inoperative. This condition exposed employees to an electrical shock hazard. (b) Outside south-west side of Welding Shop, right of entrance area: On or about 3 August 2012 and times prior thereto, the 120 volt, Ground Fault Circuit Interrupter was inoperative. This condition exposed employees to an electrical shock hazard.

Marine Terminal Forklift parking area: On or about 3 August 2012 and times prior thereto the 50K, Hyster F-32, headlights, back-up lights and rotating warning light were inoperative.
