29 CFR 1910.133(b)(1): Protective eye and face devices purchased after July 5, 1994 did not comply with ANSI Z87.1-1989, "American National Standard Practice for Occupational and Educational Eye and Face Protection."    a) St. Marys Foundry, Inc. - Saint Marys, OH: On or about August 8, 2012, employees who wore prescription glasses and flexible side shields were exposed to eye hazards such as flying particles while scraping paint and using a compressed air to clear debris. Lenses and side shields did not comply with ANSI Z87.1.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    a) St. Marys Foundry, Inc. - Saint Marys, OH:  On or about August 8, 2012, the employer did not assure that the Yale L020 forklift (s/n E177B09462S) was removed from service during a time in which it was not in safe operating condition.  Employees were exposed to injury in that the clamps that secure the propane tank on the truck were defective and pins which secure the forks were missing.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) St. Marys Foundry, Inc. - Saint Marys, OH: On or about August 8, 2012, the Cleveland Products, Inc. rollover machine, located in the C-Core area, was not guarded. Employees were exposed to pinch point and struck-by hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s):    a) St. Marys Foundry, Inc. - Saint Marys, OH: On or about September 28, 2012, the employer did not assure that lockout devices were marked to identify the employee applying the lock.  A lock without identifying information was used to lockout the mixer in the C-Core area for cleaning.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
