29 CFR 1910.219(f)(3):     Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:      a)  #1 Dryer Unloader; On or about June 30, 2012 and at times prior thereto the employer exposed employees to crush and amputation hazards in that employees were allowed to open a gate on the #1 dryer exposing unguarded chains and sprockets and use a long pole to reach into the machine to unjam it while it was in operation. On June 30, 2012 an employee while unjamming the unloader while it was in operation placed his hand between a chain and sprocket resulting in a crush / amputation injury.
