29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: a) On or about September 25, air valve for batwing: The employer failed to ensure the pneumatic valve handle for the batwing being worked on was replaced after it had broken/fallen off so it could be properly locked out during repairs. At the time of the incident vice grips were being used to turn the valve on/off not allowing locks to be installed by both maintenance personal involved in the repair.

29 CFR 1910.184(e)(2)(ii): Makeshift link(s) or fastener(s) formed from bolts, or other such attachment(s) were used to connect links or fastenings on alloy steel chain sling(s): a) On or about September 29, 2015 and times prior; canter sharp chain run out: The employer failed to ensure proper lifting devices were used during a batwing repair in that a 3 inch washer was tack welded to the batwing and connected to an alloy steel chain to lift and suspend the batwing using an overhead remote control crane. As an employee was working on the cylinder rod connection of the batwing, the tack weld broke allowing the batwing to fall onto both arms of the employee.
