29 CFR 1926.453(b)(2)(iv): Employees did not stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position: (a) Construction Site, Southern Side of Site - On or about August 21, 2012 employees were tightening bolts on structural steel beams from a Grove aerial lift and climbed the guardrails of the basket. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: (a) Construction Site, Pre-engineered Building Roof - On or about August 21, 2012 employees were landing bundles of standing seam metal decking from the top of a pre-engineered metal building and were not protected from falling approximately 20 feet to grade. (b) Construction Site, Pre-engineered Building Roof - On or about August 21 2012 employees were laying simple saver insulation on a flat roof of a pre-engineered metal building and were not protected from falling approximately 20 feet to grade. (c) Construction Site, Pre-engineered Building Roof - On or about August 21, 2012 an employee was using a piece of equipment to seal the standing seam metal roof on top of a pre-engineered metal building and was not protected from falling approximately 16 feet to grade. KEYSTONE STEEL STRUCTURES, INC. WAS CITED PREVIOUSLY FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.760(a)(1) OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 113739189, 01, 06A AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 2, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 16 MOTEL ROAD SHARTLESVILLE, PA 19554. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
