29 CFR 1926.416(e)(1):     Worn or frayed electric cords or cables were used:    a) 77 Canary Drive, Building 156, Lakewood, NJ: Employees were using a DeWalt saw, model DW 368 serial # 255019, that was plugged into an extension cord with damaged outer jacket insulation, on or about 8/16/12.     b) 77 Canary Drive, Building 156, Lakewood, NJ: Employees were using a DeWalt saw, model DW 368 serial # 255019, that had a damaged outer jacket insulation on the power cord, on or about 8/16/12.

NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
