29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) DH Steel Products, LLC / 220 Scotland Road, Edinboro, PA: On or about August 22, 2012 above the data center on the first floor, North section of existing building: A temporary knockout style receptacle outlet box was on the ground. The knockout style receptacle outlet box was used in a portable application and not utilized in accordance with its listing in labeling in that it was not designed for a portable applications and was to be surface mounted. The knockout style receptacle outlet box was utilized to power a yellow colored extension cord. The yellow colored extension cord was used to power a Metabo Grinder WA 11-125
