29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:     At the project on or about August 29, 2012, an eight foot long step ladder did not extend above the ten foot, three inch high wall for the water tank.  An employee used the ladder to get down from the top of the wall.

29 CFR 1926.1053(b)(22): Employees shall not carry any object or load that could cause the employee to lose balance and fall:    At the project, on or about August 29, 2012, an employee carried a five gallon bucket of mortar in one hand while climbing up an eight foot long ladder.  While on the fourth rung from the bottom, he handed the bucket up to an employee on the roof.  The roof was about ten feet, three inches above the ground.
