Employees were exposed to a potential fall of approximately 13 feet to the ground below, which could result in permanent disability or death.      Hardy Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 107645, citation number 2,  item number 1 and was affirmed as a final order on January 5, 2012 with respect to a workplace located at 964 Mallard Drive, Oregon, Ohio.      Hardy Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 311601900, citation number 1,  item number 1 and was affirmed as a final order on April 21, 2008 with respect to a workplace located at 3175 Sterlingwood Lane, Perrysburg, Ohio.
