29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Hardy Roofing. On or about May 4, 2022, at the site location of 9526 Rockingham Dr. Whitehouse, OH: The employer failed to ensure personal fall arrests systems utilized by employees working from a roof deck were rigged and/or adjusted in a way that, in the event of a fall, would prevent workers from contacting the ground below. Anchoring points and rope grabs were not adjusted to protect employees from an approximate 10 ft. fall to the ground below while performing roofing work on an 8/12 pitch roof.

29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Hardy Roofing. On or about May 4, 2022, at the site location of 9526 Rockingham Dr. Whitehouse, OH: The employer did not ensure the side rails of the aluminum extension ladder used on site to access the roof extended at least three feet above the landing surface. Employees in this condition were exposed to falls over 10 ft. Hardy Roofing was previously cited for a violation of this Occupational Safety and Health Standard 1926.1053(b)(1), which was contained in OSHA inspection number 1517860, citation number 1, item number 4 and was affirmed as a final order on 01/03/2022, with respect to a workplace located near 170 Valley Hall Dr. Perrysburg, Ohio.
