Employees conducted servicing and maintenance on machinery that included, but was not limited to, the Armac and Sencorp plastic cutting/forming machines.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) throughout the production areas, on or about August 23, 2012, and times prior thereto, employees conducted servicing and maintenance activities including, but not limited to, setting up, lubrication, cleaning or unjamming and making adjustments or tool changes, but were not trained as authorized employees to utilize the lockout/tag-out procedures. These employees included but were not limited to the floormen. Servicing and maintenance is conducted on machinery that included, but was not limited to, the Armac and Sencorp plastic cutting/forming machines.
