29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A.) Munot Plastics, Inc.; production area - On or about 1/29/19, and at times prior thereto, thermal mold presses did not have guarding at the mold die to prevent employees from getting in between the mold when it was closing. This condition existed on presses including, but not limited to, Browns mold press #2, #3, #4, and #5. B.) Munot Plastics, Inc.; production area - On or about 1/29/19, and at times prior thereto, thermal mold press guards had a three (3) inch gap between the cutting die and the guard. The gap was too large and employees can reach into the point of operation. This condition existed on presses including, but not limited to, Browns mold press #2, #3, #4, and #5. In addition, the back of the cutting die was exposed and not guarded.

29 CFR 1910.303(g)(1)(ii): The required working space about electrical equipment rated 600 volts, nominal, or less to ground, was in a passageway or general open space and was not suitably guarded when normally enclosed live parts were exposed for inspection or servicing: A.) Munot Plastics, Inc.; grinding room - On or about 1/29/19, and at times prior thereto, the 240 volt electrical disconnect that powered the grinders had storage in front of them. Stored items included, but were not limited to, a trash can, brooms, and a card board box of plastic regrind. B.) Munot Plastics, Inc.; by inside dock room- On or about 1/29/19, and at times prior thereto, the 220 volt electrical panel box that powered the lights had storage in front of it. Stored items included, but were not limited to, plastic sheets and a shop vac.
