29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h): a) Throughout facility: The employer did not ensure that each employee was protected from the hazards associated with unexpected start up or release of stored energy during the maintenance/servicing, or cleaning/sanitizing processes. The employer did not document, implement, and utilize machine specific energy control procedures at its facility for equipment such as but not limited to: augers, grinders, incline belts, dumpers, PolyClip and AMFEC.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect employees from hazard(s) created by rotating parts: (a) Roll Pack Out: Employees catching residual poultry products from the lower hatch of a roll pack out auger were exposed to amputations and lacerations from the rotating blades of the auger. The employer did not provide a method of guarding while the auger hatch door was in the open position.
