29 CFR 1926.451(g)(3)(i): Vertical lifelines were not fastened to a fixed safe point of anchorage, independent of the scaffold, and protected from sharp edges and abrasion:    a)  Vertical lifelines that were used by employees working from two-point adjustable suspension scaffold were attached to the hoist.  Lifelines were not protected from abrasion and sharp edges of building.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a) Employees were exposed to fall hazards while using personal fall arrest systems that were not rigged in a manner to prevent them from free falling more than 6 feet. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  Employees engaged in masonry work were not protected from fall hazards of up to approximately 30 feet to the ground, while working along unprotected edge of the roof.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
