29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      a)  Employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 28 feet above the ground.    b)  Employees were exposed to the hazard of falling approximately 28 feet to the ground while working in the hoist area.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:      a)  Employees were not wearing protective helmets where there was a possible danger of head injury.    W. E. T. Construction LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 981524, citation number 1, item number 1 and was affirmed as a final order on July 17, 2014, with respect to a workplace located at 5711 N. Kimball Ave., Chicago, IL 60659.      No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a)  Ladders used by employees to gain access to the working surface did not have its side rails extended at least 3 feet above the surface.       W. E. T. Construction LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 581338, citation number 1, item number 3b and was affirmed as a final order on September 25, 2012, with respect to a workplace located at 2825 W. Diversey Ave., Chicago, IL 60618.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
