On or about October 9, 2012, at 450 Summit Crossing Way, Cumming, Georgia: An employee installing decking on the roof of a building was exposed to struck-by hazards while using a nail gun without wearing eye protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 9, 2012, at 450 Summit Crossing Way, Cumming, Georgia: a) An employee decking the roof of a building was exposed to a fall hazard of approximately 17.6-feet when the employee worked from the roof ridge without being tied off to his lifeline. b) The same employee decking the roof of a building was exposed to a fall hazard of approximately 9.5-feet when he unloaded plywood near the unprotected edge of the building without being tied off to his lifeline.
