29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 21 June 2017, two workers were performing framing activities without the use of hard hats, exposing them to the hazard of being struck by falling materials, tools, or debris.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about 21 June 2017, five workers were performing framing activities with pneumatic nail guns and hand-held circular saws without the use of eye protection, exposing the workers to the hazard of being struck in the eye(s) by flying particles and objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  On 21 June 2017, a framer was on the first floor top plate of a residence under construction without the use of fall protection, exposing the worker to a fall hazard of approximately 9 feet 1 inch to the concrete foundation.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.  On or about 21 June 2017, two framers were using a 5-foot and 6-foot step ladders to access higher elevations in which the top step of the ladders were used, exposing the workers to a fall hazard of approximately 9 feet 1 inch to the concrete foundation.
